PARENT/GUARDIAN CONTRACT
This contract is to establish a clear understanding of Higher Elevation Youth Enhancement Organization’s (herein referred as “HEYEO”) requirements for parent/guardian of a participant who become a member (herein referred as “Member”) in the organization.
Age Requirement:
Member must be born a female and be between the ages of 9 and 18. Member must be at least nine (9) years of age or will be nine (9) years of age by the end of the calendar year in which this contract is signed.
Membership Fee:
Non-Refundable $50.00 and mandatory fundraiser participation.

- Fundraiser commitment (Must bring in a profit of no less than $150)
- Calendar sale (Must sell at least 10 Calendars)
- Attendance at workshops
- Finance extra cost of your child field trip (if applicable)
- Gala participation
Optional Webinar Package:
Online courses designed as an additional tool for parents when communicating with their child. Our online classes will introduce an interactive hands-on approach for parents. It is aimed at helping to bridge the gap that typically occurs between parent and child. Emphasis will be placed on:
- Opening the door to communication with your child
- Identifying the weaknesses/strengthens of your relationship with your child
- How to repair/improve your relationship with your child
Four (4) Webinar subscription options:
- 3 month plan for $90.00
- 6 month plan for $175.00
- 9 month plan for $260.00
- 12 month plan for $300.00 (Best Option)
Meetings:
There will be periodic parent/guardian meetings. Your presence is mandatory at the parent/guardian meetings. The meetings are necessary to inform the parent/guardian of the organization’s direction and actions, which may require your attention. You will be provided with an itinerary noting meetings and events. Communication (email, text, mail, call) will be sent to parent/guardian to remind you of the meeting and/or event.
Services:
HEYEO is dedicated to assisting parents/guardians with their adolescent. In an effort to supplement your parental guidance, HEYEO will need your full cooperation. HEYEO is committed to address your child’s academic, emotional, physical and spiritual needs. In an effort to fulfill our obligation, HEYEO may request your permission before a service can be provided. HEYEO will request permission from the parent/guardian to contact/correspond child’s educators. (HEYEO will issue a School Access form and Consent form for field events.)
Fundraisers:
Fundraising is a way to keep HEYEO operational. Each parent/guardian is required to participate in HEYEO fundraising efforts. Each parent/guardian is responsible to raise at least $150 per fundraising and if the goal is not met the parent/guardian will be responsible to pay the difference within two weeks of the conclusion of fundraiser. The fundraising drive for 2017 will take place in the months of February, March, May and November.
Attendance:
Parent/guardian is expected to ensure your child is present, on time and ready to participate at workshops, round tables, fundraisers, community services and any other activity that HEYEO support. It is the responsibility of the parent/guardian to provide transportation for your child to workshops, round tables, fundraisers, community services and any other activity unless otherwise stated by HEYEO.
Parent/guardian must pick up their child at the designated time and place. If parent/guardian is more than thirty (30) minutes late, your child will be taken to the local precinct.
Organization Dismissal:
Parent/guardian is informed of HEYEO’s zero tolerance policy and code of conduct. HEYEO will not tolerate belligerence, bullying, discrimination, disrespect, drugs, fighting, profanity, stealing, and the use/possession of weapons of any kind or any other inappropriate behavior. Violations may cause dismissal or suspension from HEYEO.
Member Requirements:
Below is a list of requirements for youth members of HEYEO:
Member must know and understand HEYEO mission statement, code of conduct, and requirements.
Member is required to attend all HEYEO’s Round Tables. Round Tables are intimate group discussions on various topics that youth deal with on a daily basis. Content of Round Table sessions cannot be disclosed due to the personal information discussed, unless approved by a HEYEO official.
Member is required to attend interactive workshops. A guest that specializes in a specific field will share their knowledge and skills with members of HEYEO.
Member is required to perform community outreach service. HEYEO officials will provide more information on the outreach services, times and dates as they become available.
Member attendance is required at other events scheduled by HEYEO unless academic or other priority prevents attendance. Proper notification will be made to a HEYEO official before scheduled event if member can’t attend.
Member will be issued a HEYEO uniform. The uniform must be worn during organizational events. Notification from HEYEO officials will be sent out regarding which uniform is to be worn at the event.
Member will abide by HEYEO Code of Conduct while present at events HEYEO is attending as well as hosting. Each member will be held accountable for her actions. Disciplinary actions will be enforced against any member who violates HEYEO policies.
Member will furnish HEYEO officials with a copy of each semester’s report or progress card. Member must maintain passing grades. Tutoring will be provided to members who require assistance.
Member will be required to complete assigned tasks by due date. If an assignment can’t be completed, by due date, proper notification will be made to a HEYEO official.
