MEMBERSHIP CONTRACT


This contract is to establish a clear understanding of Higher Elevation Youth Enhancement Organization’s (herein referred as “HEYEO”) requirements for participants to become members (herein referred as “Member”) in the organization.

Member must be a female at least nine (9) years of age or will be nine (9) years of age by the end of the calendar year in which this contract is signed.

Member must know and understand HEYEO mission statement, code of conduct, and requirements

Member is required to attend all HEYEO’s Round Tables. Round Tables are intimate group discussions on various topics that youth deal with on a daily basis. Round Table sessions are held once a month. Content of Round Table sessions can not be disclosed due to the personal information discussed, unless approved by a HEYEO official.

Member is required to attend interactive workshops. Each month a guest that specializes in a specific field will share their knowledge and skills with members of HEYEO.

Member is required to perform eight (8) hours each month of community outreach service. The services, times and dates will be provided by HEYEO officials and tailored to each member’s schedule. (Guidelines for community outreach service appear on page 2 of this contract.)

Member attendance is required at other events scheduled on HEYEO unless academic or other priority prevents attendance. Proper notification will be made to a HEYEO official before scheduled event if member can’t attend.

Member will be issued a HEYEO uniform. The uniform must be worn during organizational events. Notification from HEYEO officials will be sent out regarding which uniform is to be worn at the event.

Member will abide by HEYEO Code of Conduct while present at events HEYEO is attending as well as hosting. Each member will be held accountable for her actions. Disciplinary actions will be enforced against any member who violates HEYEO policies. (HEYEO Code of Conduct appears on page 2 of this contract.)

Member will furnish HEYEO officials with a copy of each semester’s report or progress card. Member must maintain passing grades. Tutoring will be provided to members who require assistance.

Member will be required to complete assigned tasks by due date. If an assignment can’t be completed, by due date, proper notification will be made to a HEYEO official.


COMMUNITY OUTREACH SERVICE GUIDELINES


  • A total of eight (8) hours of community service will be performed each month for a total of ninety-six (96) hours for the calendar year. Community service hours will be prorated according to the date of membership.
  • Community service projects will be performed outside of school hours
  • Projects must be pre-approved by HEYEO officials if not assigned by HEYEO.
  • A HEYEO time sheet will be submitted for all community service projects.
  • A HEYEO evaluation form will be requested from the community service project manager about your performance.
  • Members will submit a writing sample describing their duties and experiences of the community service.
  • Paid work will not be considered community service.
  • Community service credit will not be given for high school club meetings.
  • Community service credit will not be given for services provided for friends or family members.
  • Community service credit will be granted for HEYEO recruitment/membership drive and speaking at events about HEYEO.
  • Community service credit may be granted if after-school or weekend training actually result in volunteer services being provided to the community.



HEYEO CODE OF CONDUCT


  • Be punctual for Round Table sessions, interactive workshops, community outreach services and all other events attended/hosted by HEYEO.
  • Appear in proper uniform at events, maintaining your uniform in a clean and pressed condition.
  • Fighting is prohibited. Promote peace, use words to solve disagreements.
  • The use of profanity or loud discussion is prohibited.
  • There is zero tolerance for the use of drugs, alcohol or cigarettes.
  • There is zero tolerance for the use or display of any weapon.
  • Harassment of any kind is prohibited.
  • Cellular devices may not be used to make calls or text during discussions, events or workshops unless approved by a HEYEO official.
  • Disrespect will not be tolerated. Listen when someone else is speaking.
  • Respect HEYEO’s property and the property of others.
  • Be honest in your words and actions.

I have read and fully understand the rules and regulations that are required to become a member of the Higher Elevation Youth Enhancement Organization (HEYEO). I will adhere to all requirements and know that disciplinary action or dismissal from HEYEO may occur if I am in violation of this agreement. I understand the mission stated and agree to uphold the standards of HEYEO.
